Welcome to Cascade Orthopedics & Sports Medicine Center’s patient portal.

We are pleased to offer e-Chart, a secure online portal for our

patients to login and provide necessary medical history information

prior to their office visit. Patients can also access some of their

individual medical information including a visit summary online.

SIGN-UP: If you have already given us your email address, you will be

able to use the sign-up link to the right of the page to create a log in. You will be required to activate your account once you have created your log in so please look for a secondary email in order to do this.

We must have your current email address in our practice records, and it cannot be used by another patient record i.e. spouse. If we do not have your email address, please make call us at (541) 296-2294 so we can update your account.

ACCESS: With your login, you can view your e-Chart information at any

time. However, to change or update your medical information (including

history, medications, and allergies) you must have an upcoming

appointment within the next three (3) days.

NEW PATIENTS: If you have an appointment within the next 3 days with

one of our Provider, please take 10-15 minutes to

complete your Medical History form online. This will save valuable

time on the day of your visit.

RETURNING PATIENTS: If you have not completed this form previously, or

if you have any changes since your last visit, you must update the

information within three (3) days of your next visit. For example, if

you are taking any new medications or have any new allergies, medical

conditions, or surgeries then please update this information online

prior to your next appointment.

SUBMIT & SAVE: Once you are logged into your account, please remember

when updating your medical information to click the submit page button

(bottom of screen) for each tab (patient history, family history,

social history, etc) or your information will not save.
